Elderly couple living at home - Elderly couple, living together independently with need for daily assistance, meals, housekeeping and supervision for safety; both are covered under their own have long term care insurance which provides for long term care needs including home caregiver services. Involvement included working with the insurance carrier (who had previously denied caregiver service reimbursement) to establish eligibility and necessary documentation in order to access benefits through long term policy. Provided caregiver to assist the couple daily, billed the insurance carrier for services pursuant to terms of the policy; the balance of the bill for services directed to payer. Provided the services needed and eliminated all insurance and payment burdens for the couple.
Adult resident of long term care facility - Youngest resident of long term skilled care facility with permanent brain damage. Upon consent of facility with approval of family, provided a private duty nursing assistant as a personal caregiver. Receives one-on-one daily personal care in an effort to improve quality of life and provide a more pleasant environment for family visits.
Elderly client with mild dementia desiring to remain at home - Single client residing in own apartment, with no immediate family members in area, requiring supervision and personal assistance in order to remain safely at home. Provides live-in attendants to maintain household, do shopping, prepare meals and plan activities for socialization. Caregiver accompanies client to outings and activities. We continue to provide supervision of caregivers and arranges for substitute in event of caregiver's illness and/or vacation.

Young adult with catastrophic injuries residing at family home - Implemented 24 hour daily skilled care (RN, LPN); coordinated plan of care with family, Trust administrator and Court. Coordinated procurement of necessary equipment and supplies for daily needs. During a temporary family relocation, interviewed out-of-state nursing agencies and supervised plan of care while away from home. Implemented ongoing training program for all assigned professional staff along with staff from out of state agencies. Continue to manage and monitor case even when client is out of state. Since 1986, Kathleen M. Gros has continued her association with the lifestyle needs of this client and family.
Adult resident of long term care facility transferring home - Young adult resident suffered severe brain damage as teenager and had been a resident of a nursing facility for seven years. Plans were in process to transfer to the family home setting but many layers of coordination were necessary. These included assessment of patient, meeting with nursing home staff, coordinating discharge/sharing of information, meeting with family to assess proposed living space, designed care plan with 'mixed' skilled staff, procured all medical supplies/equipment, negotiated price packaging and efficient reordering delivery of products; home physician services arranged along with lab services and skilled therapy. Will continue to communicate with family and supervise staff.
